Questions we hear

Your own business,
your own plan?

About 31 in 100 San Francisco workers mostly work from home, per the Census Bureau. Owners and self-employed people here often ask:

“I own a small shop. Can I just buy a plan for myself?”

Yes. You can buy your own plan through Covered California. If you have workers, a group plan is a separate choice we can talk through.

“Is my health plan a business cost?”

It can be for some self-employed people. A tax professional can tell you how the rules work for you.

“Is it one form for Covered California and Medi-Cal?”

Yes. Covered California says one application checks you for both programs.

How we help

Owner-to-owner help
in San Francisco.

We can show California plans from several insurers next to each other. We add twelve months of premiums to the deductible, so you see a full-year number for each.

Owners often pay themselves in odd ways, like a draw one month and nothing the next. We help you estimate your 2027 profit, because that is what the help is based on.

If you have a few workers, we can explain how buying your own plan differs from setting up a group plan, so you can decide what's right for the business.

Many San Francisco plans use different groups of doctors. Two plans can look alike on price but send you to different clinics.

If you sold a business or started a new one this year, we fold that into your income estimate.

San Francisco health insurance questions.

When is open enrollment in California for 2027?

As of October 24, 2026, Covered California says open enrollment for 2027 runs from November 1, 2026, through January 31, 2027. A court case about these dates, City of Columbus v. Kennedy, is still on appeal, with argument set for October 30, 2026. Confirm the dates before you enroll. Sign up by December 31 for a plan that starts January 1.

Has California expanded Medicaid?

Yes. As of October 24, 2026, California has adopted the ACA Medicaid expansion, according to KFF. In California, Medicaid is called Medi-Cal.

Is there a California penalty for no coverage?

As of October 24, 2026, California has its own rule that most people must have coverage. If you go without, you may owe a state tax penalty unless you qualify for an exemption. The Franchise Tax Board has details.

Can my spouse who works with me be on my plan?

Yes, a spouse can be on your family plan. If your spouse is offered a job plan somewhere else, that offer can change your help.

What if my business has a slow year?

Lower profit can mean more help. Update your estimate with Covered California once you know.

Can I see doctors in Oakland or down the Peninsula?

Only if they're in the plan's network. Networks differ, so we look them up.

Do I need my tax return?

It helps. Your last return and your books for this year make for a better estimate.

Is San Francisco a city or a county?

Both. The city and county share the same borders, so your plan choices come from your ZIP code.

Open enrollment starts November 1
